Door County Public Health anticipates receiving COVID-19 vaccine in January. At this time, they are planning COVID-19 vaccination for healthcare workers not associated with healthcare systems receiving their own vaccine supply. However, they need to identify those healthcare workers to plan vaccination clinics and possible collaboration with healthcare partners in this effort. In an effort to identify those individuals, Door County Public Health is asking healthcare workers not affiliated with a healthcare system receiving its own vaccine supply to complete a survey form found here: https://form.jotform.com/203573792801054
